I just filled out Form TX, an application for
a US copyright for our new Fortran Top 90 book.
In the instructions, it says "Works published
prior to March 1, 1989 <bold>must</bold> carry
the [copyright] notice or risk loss of
copyright protection.

The document goes on to define publishing as
"the distribution of copies ... to the public
by sale or other transfer of ownership."  It
also says that a work is published "if there
has been an offering to distribute copies ...
to a group of persons for purposes of further
distribution, public performance, or public
display."

I think the standard was published (several
times) prior to March 1, 1989.
