The titles of the 17 items are listed below, "Y" indicates the "Yes" column
(without comments), "wc" next to "Y" represents the "Yes-with-comments"
column, and "N" indicates the "No" column.  Comments or reasons may be inserted
after each item or listed following the ballot.  An acceptable electronic
